Mr. Bott is a board Certified Rehabilitation Counselor, a board Certified Case Manager and a board certified Medicare Set-Aside Consultant. Mr. Bott is licensed to practice rehabilitation counseling in the State of Louisiana.
Mr. Bott has nearly 25 years of experience in private rehabilitation counseling practice. He has steadily worked in the field of rehabilitation counseling since 1975. He entered private rehabilitation counseling in 1981 and in 1988; he founded Thomas E. Bott and Associates Inc. Prior to entering his own practice, from 1983 to 1988.

Medical Document Retrieval
We will contact medical facilities - such as physicians' offices, hospital medical records departments and physical therapy and imaging facilities - as needed to retrieve medical records. This expedites medical care and testing, second opinion examinations, independent medical examinations, claim settlement negotiations and vocational rehabilitation services. Following HIPPA guidelines, medical documents are forwarded appropriately and with strict confidentiality and security by postal mail, e-mail, fax or are uploaded to a secure FTP site, on disc media or by express courier as needed.
